University Park Veterinary Patients

Welcome to the New Patient Center for new and existing patients. We understand that pet owners are looking for great veterinary care and access to quality pet health information. Our University Animal Clinic serves pets and their owners in Bradenton, Lakewood Ranch, Sarasota, and University Park. Our New Patient Center is designed to help you prepare for your first visit to our vet and to provide you with more information on our Sarasota vet and veterinary services.

We have links to several different forms that you can download before you bring your pet in to see our veterinarian and staff. You may download and complete the forms prior to your visit to our University Park animal clinic in order to save yourself time in the office. New patients should download and print the New Patient Registration Form before their first visit to our University Park veterinary clinic. Our veterinary staff requests that bird owners also complete the Avian History form since it allows us to learn more extensive information about your pet. We use these forms to manage the veterinary care that we provide to your pet as they get older and have changing needs.

Pet owners bringing in a pet for a Surgical Procedure, such as a spay or neuter, should complete the Surgical Information packet. This surgical paperwork will help keep our University Park veterinary and clinic staff current on your pet’s recent history and present health.

Online Membership Helps Pet Owners in Bradenton, Lakewood Ranch, Sarasota, and University Park

Our Sarasota vet provides these online forms to try to accommodate the busy lifestyles of pet owners. We understand that your time is important and provide the forms to reduce the amount of time you have to spend in our animal clinic before your schedule appointment. These forms are helpful for our veterinarians and staff to learn about your pet’s health history and to provide the best possible care.

We also have the Member Wellness Registration Form to allow pet owners to sign up for access to the member wellness portion of our website. The website membership registration is free and can be done online or the form can be completed and brought in to our animal hospital. Website members receive the online newsletter and have access to a library of articles on pet wellness, pet videos, and interactive games for kids.

In addition to these online forms, our New Patient Center has a section on What to Expect at your first vet visit, a virtual office tour of our University Park veterinary clinic, links to trusted vet information, information on the payments we accept, and the ability to request an appointment online.
